Exenatide is an incretin mimetic, a synthetic form of the naturally occurring exendin-4 isolated from the saliva of the Gila monster (Heloderma suspectum). Exenatide has 53% amino acid sequence identity with human glucagon-like peptide-1 (GLP-1), and has been widely used as an adjunctive therapy. Exendin-4 is a molecule currently used, is a glucagon like peptide-1 receptor agonist. Exendin-4 exerts insulinotropic effects and has multiple glucose regulatory functions through activation of GLP-1 receptor in the mammalian cells. It binds and activates the Glucagon-Like Peptide-1 Receptor (GLP-1R), thus stimulation of insulin secretion, suppression of glucagon secretion, enhancement of beta-cell mass, slowing of gastric emptying, and inhibition of food intake.
